All necessary forms should be completed and uploaded into your online registration application; they are all avaialble in our Online Registration Portal for you.

If you already have an Infinite Campus Portal account (you currently have a child who attends Islip UFSD), you can log into the portal and click on “Online Registration in the left-side menu. 

The following documents will be needed for upload to complete your child’s registration:

• Student’s Birth Certificate
• Parent/Guardian Photo ID
• Proof of Legal Guardianship, if applicable
• Custody Documentation, if applicable

You must provide one of the following as proof of residency:

If you own your home:

• A copy of the deed, mortgage statement, or tax bill.
• A copy of a utility bill, or any bill with homeowner's name and address

If you rent in a private home:

• Form A (en Español) and Form B (en Español) notarized
• Renter - Attaches a copy of the lease or rental agreement and a utility bill, or any bill with renter's name and address.
• Homeowner - Attaches a copy of their deed, mortgage statement, or tax bill.

If you rent in an apartment building/complex:

• Form A (en Español) and Form B2 (en Español) notarized
• Renter - Attaches a copy of the lease or rental agreement and a utility bill, or any bill with renter's name and address.
• Homeowner - Fills out Form B2. Needs form notarized.
